How do you work with my commercial health insurance plan for services you provide in your office?
Your insurance benefits depend upon your individual insurance plan. We do not accept payment from insurance companies, nor do we bill them directly for our services. We can provide invoices for our services which you can submit to your insurance plan. The annual membership fee may be covered by your flexible healthcare spending account (FSA) or health savings account (HSA), depending on your insurance plan. By using your FSA or HSA, you can take advantage of using pretax dollars. In many cases, the annual membership fee is tax deductible, so check with your financial advisor and insurance plan to select your best option.
Why don't you bill and collect from my insurance company?
Our membership fee grants you access and services not covered by insurance plans. We provide more than insurance companies are willing to pay for. We remain free from institutional and third-party influence with unequaled dedication to our patients and your best interests.
How do you work with my commercial health insurance plan for services outside of your office?
Services outside of our office include visits to other physicians and health care providers, prescription medications, hospitalizations, diagnostic and imaging procedures, and other services generally covered by your insurance plan. Our practice does not alter your relationship and coverage with your insurance provider.
How do you work with Medicare for services you provide in your office?
We do not accept payment from Medicare, nor do we bill Medicare for our services.
How do you work with Medicare for services outside of your office?
Services outside of our office include visits to other physicians and health care providers, prescription medications, hospitalizations, diagnostic and imaging procedures, and other services generally covered by Medicare. Our practice does not alter your relationship and coverage with Medicare.
What is special about Personalized Physicians?
We are Atlanta's first multispecialty concierge medical practice. Members of our practice have access to both cardiology and internal medicine expertise which gives a whole new meaning to primary care. In addition, you have access to two of the most trusted, caring, and capable physicians in Atlanta.
Why did you combine cardiology and internal medicine into a concierge practice?
Besides being board certified in internal medicine, Dr. Simone is also board certified in cardiology. Since cardiovascular disease is the leading cause of death and disability in this country, our practice fully understands the importance of cardiovascular disease in the primary care setting, for preventive medicine, and to pursue optimal health for our patients. Dr. Hoffman is board certified in internal medicine and has years of experience in a concierge primary care practice. Together our physicians offer a unique blend of specialty and primary care experience to benefit our patients.
Will I see Dr. Simone, Dr. Hoffman, or both?
Our patients have access to both of our physicians. One of us will serve as your primary concierge physician while the other is available to consult and assist in your care. You may choose your primary concierge physician based on your medical needs or preferences, but rest assured that both of us are available to help in your care.
Why is there a membership fee for a concierge practice?
By limiting the number of memberships available, we can spend more time with you to really get to know your medical condition, lifestyle, and needs. You have more time to communicate with us and we have more time to really think about your care. We have time to research your problems, search for solutions, and to coordinate your care with other physicians.
How do you respect my time?
We understand your time is valuable. There is minimal to no waiting before your appointment time. We allot up to 30 minutes for routine office visits allowing you plenty of time with your physician. If you need additional time, we can schedule it. If you need a same day appointment, we can accommodate you.
How can I reach my doctor after office hours?
One of our physicians is available by cell phone 24 hours a day, 7 days a week, 365 days per year. You will have our cell phone number and can call us directly.
What is the difference between the Concierge Membership and the Optimal Health Membership?
The concierge membership plan allows for a competitive annual fee with a flexible cost structure depending on which services are used and how frequently they are used. The optimal health plan includes all of the tests we offer in the office, unlimited follow-up visits, advanced lipid profile testing, a consultation with our personal trainer, and a consultation with our nutritionist, all for a single annual fee.

Can your practice save me money in the long run?
We may be able to save time, money, and exposure to unnecessary medical testing and risk. In high-volume traditional primary care practices, referrals to specialists and urgent care centers are extremely common. We have the time to carefully assess your problems and to decide when a referral is truly appropriate. Since we offer same-day appointments, many visits to urgent care centers can be avoided.
How will you work with my other physicians?
We will communicate and share records with any physicians that you select. We can exchange information on your behalf to get you the best possible care from your other doctors, and we will incorporate their findings into our medical record for your safety and convenience.
What happens if I am hospitalized or visit the emergency room?
We will collaborate with your hospital-based physicians to provide them with your medical history, pertinent information, and to help coordinate your care plan.
If I am already covered by insurance, why should I incur extra cost in your practice?
If your current insurance plan and medical providers meet all of your needs and expectations, then stay with your current program. If you are looking for extra time, personal attention, medical expertise, coordination of care, and convenience beyond what you currently receive, then consider our practice.
Is there an age limit to joining your practice?
Patients must be over 18 years of age to qualify for membership in our practice.
